Colorado

Grandcestors in Denver, CO has frozen meals available in Whole Foods in Colorado, New Mexico, Utah and Kansas, or shipped to 48 states via FedEx. Whole30 approved. There were eight meals available when I visited the site. In April 2016 Noble Savage Foods became part of Grandcestors. They had fresh packed paleo meals that are blocked out for the Zone Diet. Meals can be delivered to your door each week or you can pick them up at various CrossFit boxes in Colorado and at local Natural Grocers locations.
Caveman Chefs has pickup locations in Denver, CO area for fresh meals. They use UPS to ship frozen meals nationwide. While Paleo and Whole30 are the heart of their business, they also cater to FODMAP, Autoimmune Protocol, and Ketogenic dietary needs. [Yelp Reviews]
Paleo Meals To Go sells freeze-dried meals that adhere to the general principles of the paleo diet. (Vegetarian items have flaxseed, presumably to appeal to vegetarians that aren't eating fish.) West

Ice Age Meals is a paleo food delivery service based in Reno, Nevada. Meals are shipped frozen to all 50 states. They use fresh and mostly organic ingredients, but not all ingredients are paleo. Rice often appears, but you can skip those meals. Meals are all GMO, dairy, gluten, and preservative free. Each meal is in one container, and foods can mix. There is no plan. You order what you want and stock your freezer.
Trifecta Nutrition has Loren Cordain (author of The Paleo Diet) approved paleo plans amongst their meal offerings. Has several different plans, including a Keto plan. Meals are fresh and vacuum sealed. Free nationwide delivery once a week on Fridays. Location not disclosed, but domain is registered in Davis, CA.
Green Chef has a variety of menus, with Paleo, keto and gluten-free among the choices. Note that Green Chef sends you the ingredients to make a meal, not prepared meals. Most of the meals in this section should be fresh, but a few are frozen. Occasionally it is a mix, so a once a week delivery can last for a week.

Northeast

WholeMade Kitchen (formerly Chicken or the Eggplant) in Beverly, MA makes only fresh paleo meals for pick up or delivery. Delivery area covers many towns. Deliveries are on Mondays and Thursdays. Company name was Get Wicked Healthy.
Muscle Up Paleo has pickup locations spread over Long Island, plus they can deliver to your office or studio. Each week there are two distinct menus, each available in two sizes.
